Submitting a request to Return to Campus (RTC)

Use this form to request to Return to Campus for an occasional or short visit related to work.  Each request is valid for one visit and a new request is needed for subsequent visits.  This process does not apply to Phase 2 approved Researchers, who should adhere to their approved safety plan for on-campus work.

Step 1: Make sure you are connected to the VPN before attempting to fill out a request. 

Step 2: To begin your request, please visit You will see the screen below.

Step 3: Click on the button Login with your UCInetID.


Step 4: Log in with your UCInetID and UCInetID password.

Step 5: Fill out the Return To Campus form as shown below.

How to complete the form:

  1. Select the building you will be located in.
  2. Select the Room you plan to visit.
  3. Using the Calendar, select the date you plan to return to campus.
  4. Select the time and how long you will be in that space. You can select from as little as 15 minutes and up to 2 hours.
  5. Enter your reason for returning to campus; the department CAO will review your request.
  6. Upload a screenshot of your Return to Campus Training certificate from UCLC (under Transcripts & Certificates).
  7. Check the boxes and abide by safety protocol to keep others safe.
  8. Click Request to submit your form.

What happens after I submit a Request to Return to Campus?

  • Once the request has been submitted, your Chief Administrative Office will either Approve or Reschedule your request.
  • You will receive an email notification informing you about the status of your request.
  • If your request is approved, you can return to campus for the date and time you selected in the form. Please follow the safety protocols.
  • If your request is rescheduled, you will need to fill the form out again and select a different time and day.
